Four Checklists Families Actually Use

These are the four checklists our advisors use with Colorado families: what to ask on a tour, what to read in a contract, how to run the move, and what to do when a hospital says discharge tomorrow. Tick them off on your phone, or print one page and carry it.

Which checklist do you need first?

How do you use these on tour day or at the hospital?

Each checklist works on a phone screen and on paper, so use whichever is easier where you are standing.

On a tour, open the checklist on your phone and tick items as you get answers. You will not remember by the third community which one had a nurse on site overnight, so write it down while you are there.

At a hospital, print the discharge checklist at the nurses station or work from your phone in the hallway. The order matters: the case manager conversation shapes everything after it.

At home, print the contract checklist and mark the clauses you want changed before you sign anything. If you want a second set of eyes, call us and we will read it with you. It is free.

Yes. Every checklist here is free to read, print, and email to yourself. Our placement help is free to families too, because communities pay us when a family moves in.

Yes. Each checklist has a print button that strips the site down to the title, the date, and every item as an empty box, so it fits on one or two letter pages.

Start with the hospital discharge checklist. It is built for the seventy two hours around a discharge, and it puts the case manager conversation first.
